VNS CHOICE MLTC Plus is a combination of a Medicare Advantage plan and a Medicaid Managed Long Term Care Program, developed for individuals who have both Medicare and Medicaid. This plan is different from VNS CHOICE Option 1 and VNS CHOICE Option 2 because we recognize that not everyone with Medicare has the same needs. This benefit plan combines your Medicare and Medicaid covered services under one insurance plan, which means you would only have to contact VNS CHOICE when you have questions about your Medicare and Medicaid services or health care coverage. VNS CHOICE MLTC Plus includes many features specifically designed for individuals who have special health care needs and require supportive long-term care services in order to remain at home. This benefit has a broad range of services, which include:
- Services you receive under Medicare Part A and Part B
- Medicare Prescription Drug Coverage (Medicare Part D)
- A large network of physicians, hospitals, pharmacies, and other health care providers, to ensure that you have access to care in your local community.
- All home and community based, and facility-based long-term care services that are normally paid for by Medicaid
- Services tailored to meet the needs of people with chronic illness.
- A Registered Nurse who will periodically visit you in your home to address your health care needs.
